Hi, I’m Clare Daniels, a highly experienced Registered Dietitian and Certified Nutrition Support Clinician. I launched Home Nutrition Support Solutions to specialize in home nutritional support for patients on enteral tube feeds and parenteral nutrition.
Nutrition is the foundation of health and healing. People on home-nutrition need a lot of support to live their best lives while they heal or adjust to a new normal. Whether that support is temporary, long term or permanent, I am there every step of the way to assess, troubleshoot and to communicate with caretakers and medical professionals about their nutritional needs.
With my comprehensive background in clinical nutrition and my dedication to patient care, I am able to make a significant and positive impact in the field of home nutritional support.
I provide personalized nutrition support for patients transitioning to home care. My services include: customizing enteral tube feeding and parenteral nutrition plans, educating patients and caregivers on proper nutrition support techniques, monitoring and adjusting nutrition plans for optimal patient outcomes. Facilitating communication between patients, home health agencies, caretakers, and physicians.
With over 13 years of experience as an in-patient dietitian at a 400-bed tertiary care teaching hospital, with a Level II trauma center, I have developed extensive expertise in critical care and medical-surgical nutrition support. I have designed nutrition support plans for patients in critical care, trauma, oncology, neurology, and for those with GI disorders. As the lead clinical dietitian, I have played a crucial role in multidisciplinary rounds where I:
Communicate with multidisciplinary teams to provide optimized parenteral and enteral support plans
Calculate protein and energy requirements using Critical Care/ASPEN guidelines
Make recommendations for diet advancement and oral nutrition supplements
Develop transition plans from nutrition support to oral diets
Work with case management and social work to prepare nutrition support plans for patients going home on enteral and parenteral nutrition
Full writing order privileges for enteral nutrition and oral diet adjustments
Expertise in managing oral supplements for protein, wound healing, and diarrhea
Close collaboration with medical teams to communicate nutrition recommendations
Screening for malnutrition using ASPEN guidelines
Educate medical staff including nurses, physicians and residents on nutrition support and nutrition guidelines for critically ill patients and medical surgical patients
